Ok problem fixed, so i took the whole setup down to my supplier and here is what they did, maybe useful for others to know:

Tried many things, uninstalling drivers / m powered, deleting preferences and maudio/digi related files from library folders, etc. Also tried upgrading from 10.4.8 to 10.4.9 and also 10.4.10, using the combo update not software update, nothing worked.

Solution:
1) a Mac OSX reload over the top, choosing 'Archive and Install' option, which retails all programs, settings, and user files. This resulted in a working 10.4.7 system.
2) Loaded the M-Audio 1.8.2 drivers, and noted that midi ports now did show up!
3) did an auto software update to bring it up to 10.4.10 ( i know this isn't recommended)
4) Ran that pre-binding fix, you fix the 10.4.10 issue with tools not working after auto update.
5) Reinstalled M-Powered 7.3 and 7.3.1 patch, as Pro Tools is the only software i've seen that doesn't survive an Archive and Install load of OSX.

Fired up tools, added the project mix as a HUI surface, loaded a recent project, worked 100%.

So basically something in file system was screwed, and neither the ProTools uninstaller nor the m-audio driver uninstaller correctly removed everything and put it back. But it works now, and all files/desktop background/passwords/settings and all other apps work fine.

That's good news... it does sound like it had something to do with how AMS was originally installed. Now comes the fun part...
