Default Error 9128 - no plug ins

Hi guys. Pro Tools 9, Mbox Pro, 3rd Gen. Mac, 10.10.3 Yosemite. Trying to record a single audio track. No aux track, plug ins or anything - just a single audio track for a voice over. Done it a million times. The minute I try to record, I get the error 9128. I have removed all plug ins from the main folder in library, so none even show up in the edit / mix windows. I have adjusted my H/W Buffer size, number of processors and CPU usage limit. Nothing works. The only thing differently I have recently done was use some midi tracks (in different files - not this one, which have now been deleted) and ever since then Pro Tools is kinda wonky. Help, please?!
