Yesterday I tracked in 12 for the first time.
Finally got up the courage after having 12 since the end of Nov of 2015.

A couple of backup vocals and a miked acoustic guitar part.
Left ADC off... no blasts.

Yes, this has happened to me twice now. I'm always using Delay Compensation and mixing thru a stereo aux fader. My set-up is posted below. In the future, if I can replicate the "problem," I will. Thanks for all the comments.

Are you saying you get noise blasts with ADC disabled?

Apologies, but I'm really late for the party here and not up to speed.

I'm using 12.4 with a ADC turned on. Not had any noise blasts but really don't like the idea of hearing damage or blowing up my ATCs.

Is turning ADC off an official recommendation?

Yes, Avid says disabling ADC is the workaround in 12 to avoid noise blasts.
But I'm starting to think that the people who've had blasts are hand shy about using 12.

They don't want to hear anymore excuses.

I think it's a fundamental issue and maybe the best that can be hoped for is a patch.
Something that keeps the blasts down to a minimum, but won't get rid of them totally.

Could be enough?

Same issue for me in PT 12 HDX2. And getting worse. Horrible noise blasts through main outs and headphones. Can't use it. Musicians losing the hearing! Had to switch back to HD3 10 on Mac Pro 2009 4,1 OS X 10.8.5

I started a case with AVID about this and already returned my new HDX hardware (working on the software part next week).

To the poster above - AVID believes this is not an issue for HDX so your input to them is very important.

FWIW - the AVID tech support told me that they believe the main issue with starting and stopping causing these is fixed in 12.5. They also know about the one that involves Reverb tails and think that only effects 32bit sessions. Regardless for me - I don't have to use PT and am unwilling to take any risk with my hearing. I will happily return to PT after 6 months or so of all fixed and no recurrences.

12.5 has a lot on its plate and I will sit that one out. I would have greatly preferred a dot release addressing this instead of a whole new bunch of things in 12.5 including this. I am using Logic Pro X for the next while but will check in here now and again to see how it all is going.

Just to be clear, I'm just telling you what Avid claims. I don't know if it's true.

But since no one has come forward saying it doesn't work, I assume it does.

Well... if with that we can really avoid the blasts, my question is, can we mix a project with ADC turned off? I think no... then what is the meaning?
