Just had exactly the same problem.
I just went into "Audio midi setup" in the "Utilities" folder, selected "Pro tools aggregate I/O" or add it if it isn't there, and ticked "all" the boxes under "audio devices" in the fields to the right.
Fixed!

Problem solved for me.

The ProTools Aggregate I/O created when installing PT9 was set-up with 4 ins and 2 outs.
As the built-in i/o on my macbook pro can handle only 2 i/o, it couldn't work.

I modified the ProTools Aggregate i/o with 2 ins / 2 outs, did a clean uninstall/reinstall of PT9, and it's now working fine.

got mine running by trashing /Library/Preferences/com.apple.audio.AggregateDevices.plist

We've added both of these to Knowledge Base Article 381131:

Pro Tools could not initialize the current playback device. Please make sure that the device has been configured correctly. Error on launch of Pro Tools 9

How do I resolve the "Pro Tools could not initialize the current playback device. Please make sure that the device has been configured correctly." error when trying to launch Pro Tools 9?

This could be an issue with the Current Engine setting in the Pro Tools Playback Engine. This has been resolved in some scenarios by picking a different Playback Engine:
  1. Launch Pro Tools 9
  2. When the splash screen appears on the screen, hold down the "n" key on your keyboard, which will eventually open the Playback Engine dialog for Pro Tools.
  3. At the top of this window, you will see the 'Current Engine' drop-down menu.
  4. Choose a different Engine (such as Pro Tools Aggregate I/O)
  5. Click OK.
This should allow Pro Tools to launch.

Additional Solution on Mac OS X
If the solution above fails, you can try deleting the "com.apple.audio.AggregateDevices.plist" preference on Mac OS X. You can find this preference file in the following location:
Macintosh HD > Library > Preferences

The problem I had was you can't have more than 64 I/O selected or PT will quit on startup. I have 3 Motu 2408 MKIII. It took me a while to figure that one out.

I have two suggestions. The one that worked for me was running a program called OnyX, found here: http://www.apple.com/downloads/macos...ties/onyx.html, to repair disc permissions and a wide variety of other things in your system, It's a nice program and I try to remember to run it once a month of every other month. Once I restarted after that I was all set and running.

Simple Fix
