Default Audio Clock issue

Whenever I close one session and open another, I get an error message as soon as I press play. It pauses for a second and then tells me there is an audio clock error and I need to check the audio clock and the sample rate. I can't see anything wrong with them (I always leave it on 'internal' as I have no external stuff to keep in sync). If I go to the hardware settings and try to change the HW buffer, I'm told that PT cannot reacquire the hardware as another programme is using it. Then it tells me that PT must close due to an unknown error and that PT is not installed!

Any ideas? I have made sure no other audio apps are running (as far as I know) so I don't know what the issue is.

System is in signature. I now use PT10.0.1 and Mac OSX 10.7.3 although this issue started when using PT10 and OSX 10.7 so I was hoping that the upgrades might put things right somehow.

Mac OS X 10.7.3 (Lion)
Pro Tools 10.0.1
