AudioVision Ver 3.6 - Problem on CH 1 & 2

I have an AudioVision 3.6 and somehow all the audio channels are playing back except channels 1 and 2. When playing the calibration tone from the AudioVision Meter Tool it shows the proper tone levels on this tool, but no tone is seen or heard on the 888 I/O interface.
