Default ProTools 6.4 - INTERMITTANT STUCK NOTES!!!

I use PT for my MIDI sequencing. After moving from 5.3.1 to 6.4 (TDM) within the last 2 weeks, when I create sequences, I experience random intermittant stuck notes when playing back or recording MIDI using my outboard gear (Karma, XV, etc.). When I use my internal MIDI gear ( Indigo, Kontakt) there are no problems. The new 6.5 CoreAudio driver doesnt seem to improve this situation. And trashing the Digi preferences file doesn't seem to help either. Is anyone else having this problem? And, if so, how did you fix it? Im THIS close to just buying DP4 and not dealing with DigiMIDI anymore...Thanks!
