Default DAE error -1146

Running OS 10.4.11 on a Mac Pro Dual 2.66GHz Quad-Core computer with Pro Tools HD 7.4. and HD3 system. One PT HD Core, two HDAccel cards (PCIe versions). 192 and a 96i/o (via daisy chain from 192) connected to the Core. Single 96i/o connected to the first Accel card.

My initial PCIe configuration was as follows:
Slot1: Video card
Slot2: HD Core
Slot3: HD Accel (lets just call it Accel-A for clarity)
Slot4: HD Accel-B

it worked like this fine for one day, then on the second day, as Pro Tools was starting up, it freezes at the Initializing Hardware stage, and displays DAE Error -1146. I restarted the Mac and cycled the I/O's, trashed all the relevant preference files, all the usual fixes, but with no luck. Even with the I/O's disconnected, I continued receiving -1146 errors, so I then turned my attention to the cards. Here is what I discovered:

In these configurations Pro Tools launches perfectly and is stable.

Slot1: Video card
Slot2: HD Core
Slot3: HD Accel-A
Slot4: Empty

Slot1: Video card
Slot2: HD Core
Slot3: Empty
Slot4: Empty

In these configurations Pro Tools crashes on start up and gives DAE error -1146.

Slot1: Video card
Slot2: HD Core
Slot3: HD Accel-A
Slot4: HD Accel-B

Slot1: Video card
Slot2: HD Core
Slot3: HD Accel-B
Slot4: Empty

Before I give up and send back Accel-B, can anyone offer any advice as to perhaps something I missed. I'm still unclear as to the exact meaning of -1146 errors, so if anyone can clarify that it would be greatly appreciated. All feedback is welcome. Thank you for your time.

Default Re: DAE error -1146

Please try running Digitest - it looks as if there's something wrong with Accel B and Digitest will likely give more info on the failure.
