Protools quits when making new session

Protools quits when making new session. The new session dialog comes up and after a few seconds the whole thing quits. Then I get an System Error type 2 message.

Anyone had this happen?

Recently we have had this happen to us on almost every session. We tried all the recommended tricks (rebuild desktop, zap PRam, trash prefs, reinstall apps). We even sent in our Mix Core Card because is consistantly failed DigiTest. Somewhere in the taking out and putting back of the cards the problem (largely) went away. We also replaced a really long TDM Cable with a shorter one. We suspect it was hardware or cable related. Hope this points you in the right direction.
