DAE error - 9060 was encountered. Help????

DAE error - 9060 was encountered. Help????

I keep getting this error when I try to record. I have four xlr's in the rack and one 1/4inch in the 5th analog input. It happens within the first 3 seconds. Can anybody please help me???
