It's included in the MOTU midnam distribution, which you can download from: http://digicake.com/midnam/index.html
If you have trouble using the midnam, run the MOTU folder through CherryMangler - it will convert the entire MOTU folder into a ProTools compatible one. ( the link to the download is at: http://alterspective.com/cherrypicker)
