Default running 7.4 on OS tiger on a mac pro

I went to this when the mac pro was first introduced. It has been rock-solid for as long as I can remember. Perhaps stupidly I rewired my control room (I had been adding gear on an adhoc basis for years and it was just time it was taken back to basics and rewired) last week and strange stuff has been happening ever since. The nub of the problem is that while my 192s show up in the setup i/o and I've assigned my pres to inputs accordingly, only 12 out of 16 pres then show up under the input assign button on the channel strip in mix view.

As a small experiment I deleted a pair of the inputs that were showing up on the channel out of the setup i/o window. They disappeared from the channel strip input assign. I then created a new path and added them back in the setup i/o but they didn't show back up in the channel strip input assign. Eh?

I've been using PT for 10 years and count myself as being pretty good at troubleshooting but, on this one, I'm stumped. Any ideas?

This is likely too basic, but you mention a "pair" of inputs for mic pres. Are you making your paths mono or stereo as needed to correspond to a mono or stereo track? i.e., a mono track won't see a stereo input path.

And this is confusing:

Yes. Have been doing this for a long time. This is something weird. I dont get why there can be a mismatch between what setup i/o is saying and what I am seeing on the channel input routing. Will take some photos tomorrow.
