You 1/2 insert path is overlapping with your audition path. If you are going to use 1/2 for inserts, then take it out of the output path (monitor/audition).
Best practice is output/input page doesn't use anything you are using in inserts (delete or uncheck the box.)

But it's not when he uses 1-2 for inserts, it's when he uses other inserts and 1-2 as outputs....

Ahhhh a good old fashioned reset of PT Prefs has resolved all of the above issues...thankfully!
