Have you trashed Prefs? ... use Pete Gates PT Prefs utility and trash everything.

I trashed prefs with still no luck. So I did a system software restore. Still no luck. I’m totally out of ideas now

System restore how? Using the recovery partition and wiped everything? Then reinstalled all your apps? Or from TimeMachine? <- that's not a good test. Or something else?

I would have done a manual new install onto a spare container/partition using an installer downloaded from the Apple store. Not touched your current boot partition at all. I would use new downloaded app installers etc... extreme paranoia... you are just trying to prove it works at a baseline level (hardware might be faulty, there might be some deep bug a vendor needs to look at (and you have shown that with these tests on a clean system).

Actually I would do a full clean install of MacOS then install Pro Tools and confirm stuff works with headphones as the playback engine then try installing the UAD drivers. Just to KISS and be paranoid, even if I have no explanation for what is happening.

It is very hard to imagine how this happens, especially on a full new install. If you have tried that and done full reinstalls of Pro Tools and UAD software then who knows, it's time to contact UAD and/or Avid support. If things like headphones do not show up still as a Playback Engine option then the issue belongs to Avid and/or Apple, So start with Avid support. Posting your incident number here might help you get attention, or help others who run into this.

Thought I'd post an update on this as I found the culprit. I have a LG Ultrafine monitor that connects via Thunderbolt. I removed this monitor and now all is back to normal.
