We just encountered this error with a new system installation of ProTools HD2 and ProTools 9 in a Mac Pro Quad 2.66 GHz.

The system was working fine on the test bench, but when we moved the Computer and 192 i/o into the studio we got an error - 12004 when starting ProTools and ProTools then shutdown. We ran Digitest it said all was fine. After the usual deletion of preferences, re-seating of cards, cables, and a half dozen other things still no luck. Although it felt like a hardware problem to me, we tried reinstalling the ProTools 9.06 updater again and then we were able to start ProTools without the error.

We spent considerable time on this and could not find any other references to the error - 12004 on this site or via Google that have any resolution, so hopefully this saves others some time in the future.

I have no explanation at the moment as to the sequence of events that caused this strange - 12004 error message. Hopefully we will not see it again.
