192 output card not being recognized

Has anyone ever had a problem with a 192's output card not being recognized in the Hardware Setup / IO Setup? We recently installed an additional 192 (eBay purchase) into a rig and the interface's Input card is being recognized in both of these setup pages, but not the output card. This is not in the expansion slot, but in the dedicated output card position. I popped it open to check the ribbon cable - it looked fine, but I reseated it just to be sure. At the end of the session today, I'll probably end up pulling one of my other cards and putting it into the new interface just to confirm it works before I assume the worst (bad card / something in the interface). This can't be software related, can it? I don't think so, but you never know.....

I have seen failures where the card is just not recognised. Had to return cards to Digidesign for replacement.
