Default Crash when opening sessions

Sometimes when I'm opening a session part way through the set-up I get a dialog box that says, "Could not complete your request because:Access violation occurred" and the computer crashes. It only happens about half the time. Any suggestions?

Please start with the 'General Troubleshooting' steps in the sticky at the top of this forum, specifically those areas on trashing prefs and database files.
