Default Midi i/o

My question? I'm running Mac Pro early 2009 OS 10.8.5 on Pro Tools 10.3.10. I have racks of MIDI sound modules i.e. Fantom XR, Korg Triton, etc. My MIDI patch names work perfectly in Pro Tools 10. Does anyone know if I can call up the MIDI patch names if I upgrade to Pro Tools 12 with the latest Mac OS. Will my Digidesign MIDI I/O work? I'm not all in the box, I stem through an analog console. I love my system but software upgrades are beginning to fade. Anyone have any solutions or know anything?
