Pro Tools HD 8 - Not seeing I/O

Hello All,

I'm using Pro Tools HD 8.0.3 with an Aphex 142 ADAT. I running OSX 10.5.8, 2.8GHZ 8-Core, 18GB ram. In setting up new sessions Pro Tools is only seeing the interface's 1 and 2 analog inputs. I've always had 1 through 8 and the ADAT ins. I cannot get input into Pro Tools on inputs 3 through 8 or my ADAT inputs. I can hear my source on the stereo buss from the console (Amek Big 44) but not when i monitor back from Pro Tools alone.

I've re-started Pro Tools, re-stared the computer, toggled the I/O settings within Pro Tools, etc...

Does anyone know how to resolve this? I appreciate all input. Thanks.
