Default Input states <ERROR ENCODING>! Please help! Mbox (3rd Gen)

Have MacBook Pro (early 2011), PT 9.0.6 running on Lion, the latest Mbox (3rd gen) driver, and my Mbox won't work! Have clean reinstalled and even reformatted the Mac completely! Problem with the driver I think. The input states <error encoding> and often crashes if I try to fix it. I need someones help, please! Avid support in Australia is hard to get....

Can anyone help?
