One D-Command fader pinned at top

One fader on my D-command has always been a bit funky. It worked but didn't playback fader moves. However when I turned the console on the other day, that fader went all the way to the top, did not display levels in a session and utility/test/fader recal didn't fix it.

I replaced the fader , but the new one did the same thing. Fader does not appear to be transmitting level to Pro Tools so it is essentially dead.

Any suggestions for a possible firmware reset or other thing I can try ? System is 24 chls, and all other faders are working fine.
