Default Session audio drops and crashes

I'm gonna edit this post from awhile back and bump it up since I still haven't solved my issue.

Does anyone else have this issue? I hit play and, seemingly at random times, the audio drops with a loud thump, followed by this thump "echo-ing". Closing/reopening the session will stop it until the next occurrence.

I'm using a MacPro 12 core, 12 GB RAM, Mbox Pro (3rd generation, firmware updated), PT HD 11.2.1
System drive is an OWC 240GB SSD (6GB) and audio is a Western Digital Black 7200 RPM 1TB.... both internal.

I've tried:
• a total clean install of Mavericks and PT 11.
• installing no 3rd party plugs and only working with the bare minimum of default avid plugs

I've had it happen both with an hour long live recording of around 30 tracks and on sessions as small as 3 or 4 tracks. Session size or drive don't seem to matter. Like I said, it happens without warning and when stopping and starting playback; usually when repeatedly playing a section from the same point (like loop playback to eq drums or something). I've had the problem since PT 10 through 11.2.1. I thought it might be my "unsupported" 002R. So using both a borrowed Mbox 3 Pro (firewire) and a Scarlett 2i2 (USB), I still get the same results. This is happening several times a day and is killing my workflow. I'm not sure where to troubleshoot next and would greatly appreciate some advice.

I've had some weird drop outs. Only 11 though and the sessions are on the bigger side.
