I am having the same issues on 10.2. I had been on 10.3 but rolled back because of another issue which may have been unrelated.

All my sessions are 44.1kHz, I generally run 6DSP/192 voices and a lot of Echoboys.

having the same problem on 10.3.3 in OS X 10.8.2. really annoying to have to quit pro tools, and open up pro tools again to go between sessions. doesnt happen with every session, but often its the case

Have you tried assigning more DSP's per voices in the playback engine?

right now i have it set at 192 voices (6 DSPs) with an hd3 rig

It's been a long time, but did anyone ever figure this out? I have the same problem. It is apparently associated with Accel plug-ins. Any work-arounds or solutions are greatly appreciated.

It's been too long for me to remember what fixed it but I haven't had this problem since going to one of the later 10.3 releases. I am currently on 10.3.6.269 and it is running stably.

Are you running SoundToys TDM plug-ins? If so make sure you are on their latest version.
