Default Two signal path questions

1.
I feel like I should have determined this long ago, but what is the signal path on the insert section of a track? Does it go top to bottom or bottom to top?

2.
Does PT ever limit the number of bus paths?
I recently discovered the feature of renaming a bus path, which, in most of the sessions I've been working on recently, also creates an entirely new path while leaving the original path free, i.e., if I rename "11-12" to "Gtr Verb," then I get a new bus path without losing "11-12."
But in one session, that's not the case; I seem to have run out of paths. A few of the bus paths still have their default numbers and the rest have custom names. The total number of bus paths I have access to there is 22, or 11 stereo pairs.

I'm using the 2018.4 version.

1. Top to bottom. Limited to 10.
2. Afaik no, as soon as you default the i/o the list will repopulate the busses no renamed starting on 1-2 again.
