Does this happen all the time or did it happen just once?
Were you using any plug-ins?
If you were using plug-ins or VIs, what was loaded at the time?
What audio interface are you using?

What were you doing when you received this error?

Are you using the latest version of Pro Tools 11?

We need to know what interface.

Are there any other peripherals connected to your computer? (keyboard, tablet/phone via USB, ect...)

I don't know if this will work for you, because I don't now why it works for me, but I am using the WD External hard drives, and when I get Access Violation errors, DEP errors (I think it's usually -6021), it seems to have to do with the way PT and these WD drives communicate. Maybe it's too big at 4TB, I don't know.

Anyway, I'm able to open PT, but then opening a session results in these errors. So I open PT, but don't try to open an already-existing session; I create a NEW session, name it, save it, it opens, then I close it, and whether I close and reopen PT or not, the sessions I want to open will open again. The error is gone until some indeterminate point in the future, at which case I do this and it fixes it.

Again, don't know why this is...

I'm not using WD externals, but WD internal drivers.
I'm having issues with loading pre-existing sessions, the sound and VI patches are all garbled (the playback with DAW error -6021 or similar).

Access violation occurs sporadically.
More common is DAW -9173.

Using the Supreme X-Fi III right now, no interface.

Would an interface over Supreme X-fi drastically increase performance?

I use 5.1 cans for mixing. So I'd need an interface with at least 6 channels output analogue.

I'm assuming you are using the Supreme X-Fi with ASIO4All?

This could be causing instability on your system since the audio card is not ASIO and the ASIO4ALL driver is just kind of a work around.

At a minimum, make sure you are using the latest versions of drivers.

And in my opinion, yes, a decent quality dedicated audio interface would go a long way to improving system stability.
