Replacing a Lost iLok

Hi,
Lost my iLok a while back and bought a new one, and I'd like to try and get my Pro Tools license replaced. When filling out this form I'm getting the dreaded "Error trying to create lost iLok case. Please try again later or call the Customer Support." message. I've followed all the other steps of filing an RMA, buying an ASC, and I'm entering the serial number of my old iLok.

Anyone know the solution here?
