No output to 1622 *and* master fader shows output?

I just got a 1622 and am having some difficulty with it.. any help would be appreciated...

When playing back a score/waveform/whatever, my master output fader shows output, however the 1622 does not seem to notice it. Furthermore, when i put a valid input signal on one of the 1622 inputs, the PT mixer channel monitor does not show any input. My hardware settings are setup as such in the hardware config window:

mix card 1:
interface port - A
sample rate - 44.1k
sync mode - internal
ch 1-2 - analog

btw this is a 24/mix+ system on dual g4 500 mac.

-Reseat the interface cable, at both ends, checking for bent and/or broken pins

-Trash the DigiSetup File from the System Folder>Preferences folder, empty the Trash and restart the computer. When you go back into Pro Tools, do so from the Pro Tools Application Icon and not by double clicking on a session. Once Pro Tools is launched, reconfigure the hardware settings.
