Default Weird error - PT plays back another file when importing from Sound Miner or Finder

Hi,

I'm having a very strange error during playback and editing in PT 11.3.2 (OS X Yosemite 10.10.5 ) when importing from Soundminer HD v1.0 or directly from the Finder.

When I drag and drop a file from my library, through Sound Miner or Finder, into the Pro Tools session, it quickly changes the name of the file into one of the files from the production sounds (I'm doing sound design for a film). When it changes the name, it also trims the file and plays back the production sound file. If it's a stereo file, it only does it with the one on top.

Does any one know why can this be or experienced something similar and had a solution?
