Default Error 7401 when using Slate Digital FG-X

Guys,
I am getting a DAE 7401 error when inserting the Slate Digital FG-X on any channel.
I am running Mac OSX 10.6.4 intel - Snow Leopard. I thought it might be because I am running a high track count, but I started a new session and with just one audio track in the session, I am still getting the message the minute I insert it.

Has anyone else seen this error? I have blue tooth and airport turned off.

I am using Pro Tool M-Powered 8.0.4 with all the very latest updates.
