Yes, do a clean install of Sierra (not upgrade, but erase hard drive and start fresh) and then install PT 12.6.1

This may be a dumb question, but I can't seem to find the 12.6.1 installer. I only see the 12.4. My Annual Upgrade plan expired. Does that mean that I have to buy another subscription? Is it worth moving from 12.4 + Mountain Lion to 12.6.1 with Sierra?

EDIT: Found the link to renew the upgrade plan for $299... seems pretty steep. Can I run 12.4 with Sierra?

EDIT2: I tested out Sierra quickly with 12.4 and a few of my projects. Everything works out perfectly fine. Actually impressed with the numbers I was looking at. I didn't even do a clean install. I just upgraded my El Capitan partition to Sierra and it brought in PT12.4. Granted, most of my projects are 6-10 tracks with 2-3 plugins per track each and only Addictive Drums 2 as a VI. Even so, the CPU meter was at 8%. I think I'll stick with 12.4 unless there's a compelling reason to pay $300 for the upgrade.

FYI I'm running the following:
MacBook Pro 9,1 with an OWC 3G SSD
2.6Ghz i7, 8GB RAM
OS 10.12 Sierra
Digi 003
OWC FW400 external HDD

Just updated a few more things.... everything is running very smoothly with the existing install (not a clean install), but I will wipe this drive and install the 6G when I receive it. That should help as well. I think I may have a super machine now that I'm upgrading RAM to 16 and SSD to a 6G. Just hope 12.4 is good enough for what I need.

You can, but it won't be supported. Your mileage may vary. First release done with Sierra in mind was/is the 12.6.1 that just got out
