Renaming region and disk file - line 1242 error

Name region and disk file returns -

'Neo Assertion in... line 1242' error

Naming only the region works fine, but that's not what I need. This happens when I try to rename any region. It just started happening in 9.02 so I upgraded to 9.03 and I get exactly the same error.

I'm baffled. Any clues?
