S1 Sends jumping on their own

I was working on a song and suddenly the reverb went crazy on an acoustic guitar track. I was not doing anything with sends at the time, but something caused the "Hall" send to jump up suddenly. I then noticed the LED on the S1 doing a little dance. "gt ribbon.01 and gt cond.01 are grouped. I have all the send levels grouped as well. Notice in the video that the track volumes move together as they should, but the send level is completely erratic.

Turning the S1 off fixed the issue but as soon as I turned it on again, "Hall" started flashing again on the LED and the two sends did not move together.

Closing the session and reopening did not fix it.
Quitting Pro Tools and reopening the session did not fix it.
Rebooting didn't fix it.
The problem stays on Fader 4 if I bank.
The problem stays if I open a different session.

Now I realize "pan" flashes the same way.

So it could just be a hardware issue with this unit.

How should I proceed, Jeff?

Can you run through each encoder and fader in the Test Mode and let me know what you find?

Bank L + Bank R + Nudge R while powering on.
