Where's My Free Pro Tools 11 Upgrade?

If you believe that you should have received Pro Tools 11 licenses based on a recent product purchase (either upon initial product activation or via an instructional e-mail from us) for a product, but have not yet received either the licenses in your iLok account or an email with instructions on how to do so, you may contact us through our registration case page here:

http://avid.force.com/Support/RegistrationCaseCreation

In the question box, please enter "Pro Tools 11 Upgrade". Then enter your remaining information. If this is for a boxed Pro Tools product that included a pre-programmed iLok key, please attach a copy of your scanned purchase receipt using the browse file box at the bottom.

Once your registration and/or purchase has been verified, we will send an email to you with instructions on how to obtain the Pro Tools 11 upgrade license and downloads.

Please include any and all details that may help us help you.

Where do I found my serial number or activiation ID? Is this for my current version of Pro Tools? I also should have gotten an upgrade email.

I have just tried the case form link above, however when I click 'send' it takes me to 'force.com' and tells me:

"Authorization Required

You must first log in or register before accessing this page.
If you have forgotten your password, click Forgot Password to reset it."

I Purchased PT 9 Student in 2011 and received my free upgrade to PT 10 upon release. I have been checking my email and have yet to receive my upgrade to 11. I have filled out the above form however I no longer have my activation card can you inform me as to how to obtain my serial number? I do have my original purchase receipt.
