error 13001 when launching OMS

I.ve never been able to get midi working on a G4 as 'error 13001' appears when I launch OMS.The OMS program still shows up and appears to exist but nothing can communicate using midi.

Reinstall OMS. But first, trash the following before running the installer:
From root level of the hard drive:
-Opcode folder

From the System Folder:
-OMS Folder

From System Folder>Extensions folder:
-Open Music System
-USB OMS MIDI Driver

-After installing OMS, drag a fresh copy of the Digi001 OMS Driver into the System Folder>OMS Folder from the System Items Folder on your PT LE CD-ROM.

-Launch OMS and create a new Studio Setup.
