stolen iLok, ASC not working

Hi.
I had my iLok stolen, submitted RMA at ilok.com, succesfully got my replacement licences for iZotope, Sonnox and SynchroArts plugins. Thankfully waves don't support iLok anymore.
Now it's the difficult part - Pro Tools and Complete Production Toolkit licences. FAQ stated that I should fill the form (http://avid.force.com/Support/RegistrationCaseCreation) but it didn't work. Login error.
I thought I had to buy ASC to get help (this is stupid, but okay, that's the cost of having my license back). I bouht ASC, filled another form, explained my case, attached scan of police report and after hitting "Next", I got force error message:

Error: Error occurred while loading a Visualforce page.

I don't know what I have to do to contant anyone at avid to help me in this case when all I got is forms and errors. Is there any human working there?
Oh, I didn't want to solve this by the phone since I have to send copy of police report anyway by mail (and maybe I'd have to buy another ASC to do this, I don't know).

Other developers helped me within couple of hours so I guess it's not that time-consuming and technically difficult.
