Curious to know @smokeydan: Which version of Mojave are you using? Have you installed any updates? I'm looking at the update history of High Sierra (which previously worked for me last november, but now it doesn't) and I'm seeing a couple of updates related to the IOThunderboltFamily kexts - which might have to do with the firewire connection, among other things...

I'm also booting Windows 10, and I can connect to the MBox over there... but not on macOS - HighSierra or Mojave.

From the "System Report", looks properly connected & identified:

I'm beginning to think this is an issue with the Drivers and recent updates on HighSierra & Mojave (look for "IOThunderboltFamily"...):

- https://support.apple.com/en-us/HT211100

With support ending several months ago, I think we're out of luck.

I tried the "re-install Mbox drivers while connected" thing, and it finally looks like it's working in HighSierra 10.13.6:

Sorry for the delayed response...

The Mojave installer was 10.14.6.06. I believe I downloaded and installed whatever was the current update as of March, but I don't have it to hand to check

Just adding my prior experience. It's been a while since I stopped using either Mac OS or MBox 3.

Every single time I ever upgraded my Mac OS version I needed to uninstall and reinstall my MBox 3 drivers manually to get it to work after the OS upgrade. Just standard for the MBox drivers and app I guess.
