Inserts unavailable: output paths already in use

I'm running hd native with an omni and hd io on a dual 2.4 8 core westmere os10.6.4 pt9. I continually get an error when trying to use inserts stating that output paths are in use. They're not. I can even turn them off in the io so they can't be used. Restarting makes no difference. The workaround is to delete and recreate the inserts in the io window.

Can anyone else replicate this bug?
