PT 12 perpetual licenses
I had two Protools 12 perpetual licenses. I surrendered one for a yearly subscription paid monthly. Can any one say whether I still have access to the next license? Or can I surrender that one too for a subscription?

Why not? Check your iLok account, if you have a permanent license there then you have a license to surrender.
