Default Tracks/Layouts not displaying properly when opening sessions

We are experiencing a bug where one of two things can happen (seemingly at random):

1. Session opens and tracks do not display properly (please see photo attached).

2. Session opens and Layouts do not recall to faders (all faders and knobs go blank on layout recall - switching back to regular All Tracks mode works fine).

Greetings,

Are you experiencing these issues with current versions of Pro Tools, S6 and macOS? Also - is this a new or legacy session, template - or did it originate in Pro Tools 12.x?
