If you on Mac, launch "Audio MIDI Setup" to ensure that device recognized by OSX correctly (screenshot, something like this http://farm3.static.flickr.com/2541/...d4c187c2_b.jpg).
Use "s n o i z e : MIDI Monitor" software for Mac or "MIDI-OX" software for Win to ensure that your MIDI device sends MIDI messages (notes, CC, pitch ...).

also make sure you turn on the device before starting protools and also make sure no other programs are using it

May be "main Menu -> Options -> Midi Thru". Must be checked...

BTW: Are you using "Direct Link"?

"Peripherals -> MIDI Controllers" used to configure "Remote control surface" such as
  • Command|8
  • HUI
  • MotorMix
  • CS-10
  • PC-1600
  • Surround Panner
  • M-Audio Keyboard

"M-Audio Keyboard" driver used to control Pro Tools (mixer, plugins, sents, etc) using M-Audio keyboards with "Direct Link" and "Hypercontrol" features on board. Appropriate MIDI device (such as Axiom Pro) has dedicated MIDI ports which are compatible with "M-Audio Keyboard" driver.
So, "Peripherals -> MIDI Controllers" used to control Pro Tools, but not for playing and recording MIDI.

Suggest you to remove device assignment from "Peripherals -> MIDI Controllers" and setup I\O for MIDI track one more time.
