003 randomly goes into shuttle mode

Hey DUC,

Pro Tools 10.6.7 goes into shuttle mode randomly when using my 003. It truly does seem random, some days it'll happen once and others it'll be every few seconds. When running it from Aggregate there is no problems. It happened on 10.6.5 and 10.6.6 also.
I've clean uninstalled PT and changed OS from Lion to Mountain.
I've changed Firewire cable (2m 400-800).
I've updated 003 firmware and drivers (10.3.3 driver).

Another problem also occurs randomly: the command, option and control keys become zoom in buttons!
