Default audio feedback loop when previewing plug-in

I am getting an audio feedback loop when previewing (the preview button) native plug ins. This never never happened with PT 5.1.3 but it seems to be happening with PT 6.4.1. It doesn't inhibit operation in WAVES X-noise, for example, -- I have to hit both preview and learn in that case -- but when I preview I have to kill my output so the electronic feedback doesn't annoy. This is most puzzling. I took a look at my I/O settings. I can't see where this feedback is coming from. Anybody ever experience anything like this?

thanks
