11.2.1 install issue (Mac) [RESOLVED]

Anyone seen this issue when installing the upgrade?

Could not create a new document because Assertion in "/Volumes/Development/139084/ProTools/DFW/MacBuild/../Views/UMenu.MacOS.mm", line 1549.

I am thinking that it is a 10.9.4 issue?

correct driver has been installed.

Any advice would be helpful,

Have you tried holding the "N" key while launching Pro Tools? I had a similar error, and holding the "N" key allowed me to toggle (not change) my playback engine and then the error did not recur.

Holding down the "n" key has reset the issue.
