Default Crashes when recording stopped

When recording my session will immediately crash when recording has stopped.
I created new session and tried to record a new track with/without audio, midi, plugins, various sample rates/bit etc... either way it crashes and nothing has been recorded.

I have tried most of the suggestions on the forums with no luck.
Any help is greatly appreciated!
So far I have:
-Deleted pro tools preferences
-Repaired permissions
-Deleted databases and volumes
-Downloaded latest drivers
-Was unable to reload pro-tools because I lost my disk and autho code!
I am considering upgrading but not sure if problem will still exist.

Mac Book Pro OS 10.4.11 intel Core Dup- 2GB DDR2 SDRAM
M-Audio Project Mix I/O
Pro Tools 7.1.1cs1
External Fire wire Hard Drive - 57.27 GB/ 45.19 free
