I also have not received my PT 11 upgrade code from PT 10 Educational which I purchased and registered before April 7th 2013.

Your records show this registration:
Product: Pro Tools 10.0 for Institutions Purchased: Feb 15, 2012 Registered: Feb 15, 2012

Which would not qualify for a free upgrade. If you believe that you should be registered for a Student version rather than and Educational Institution version, you would need to contact Avid Support. See the first post in this sticky thread with instructions:
