Reinstatement Plan disappeared from account — Resolved

I purchased Pro Tools Ultimate Reinstatement plan through Avid web store about a year ago. I DID NOT USE IT! It was sitting in my account some time ago, I checked. Now it disappeared, and Avid just tried to charge my credit card with $399, supposedly for some subscription that I didn't order. I have two perpetual HD/Ultimate licenses in my account. Is there a way to contact support? I tried contacting store support at https://shop.avid.com/ccrz__CCPage?p...=ContactUsForm but form doesn't send with ERROR : Please enter value for all the mandatory fields. Probably because form doesn't let me select anything in "Secondary Case Reason". My order number for reinstatement plan is ---
