Default Play back issue

I have been troubleshooting a playback issue after a new install of Pro Tools 11.1.2 (Win).
Anything I try to play back through MBOX USB Connection causes a choppy sound (seems to be using the wrong sample rate).
I have tried creating and play back at 44.1 and 48.0. (ensured mbox control panel matches on playback)
I have created a new track imported a clip, recorded a new clip, just adding a midi note played through expand2 will create the issue.
I have enabled/disabled Windows 7 system sound just to test. (same issue with windows sound too/keeping off with forum recommendation)
I have gone through and optimized windows 7 as per avid recommendations.
Installed, uninstalled, and re installed drivers with multiple reboots.
Searched forums with no luck. (but still searching)
I am very new to pro tools and I am most likely doing something stupid, but cannot figure it out.
The workstation is brand new hpz820 just received yesterday, and I did not have any issues on my last PC.
I also tried 2 usb3 external drives to save projects and the system drive. (I know the system drive is wrong but I wanted to rule out a USB3 issue)

MBOX 3rd gen I believe it is called. Not the pro and not mini.
Blue Screened after ASIO4ALL.
I am going to try a system restore than attempt PT install again.
Any other Ideas are appreciated.

One thought here. Since you have an Avid interface, and Avid has drivers for it, there is no logical reason to install ASIO4ALL on your system(it hosed my 003 setup every time I installed it). First thing I would do is remove ASIO4ALL and then re-install the latest Mbox driver

The workstation came with oem windows 8 and 7 discs. I tried the windows 8 this time and all is good. Not a fix but work around.

ASIO4ALL is not the way to do things with the Mbox 3rd gen. That interface is super stable on Windows 7, I had one for years. I recently installed it on a friend's mobile PT10 rig, and it works flawlessly there as well. Any problems getting it to work can usually be chalked up to user error. ASIO4ALL is only intended to provide a workable ASIO-compatible driver for devices that only have WDM drivers. The Mbox does not fall into this category.

One thing that can cause issues (either messages about sample rate problems, or audible issues) is when you have the Mbox set as Windows' default playback and/or recording device. Sometimes this works fine (worked as default for me with Pro Tools 10, but not with Pro Tools 11) but it is not the recommended way of setting up.
