I would say this might be happening if you're not waiting long enough for your 16a to boot before opening PT. It takes about 30 seconds to boot up, so make sure the display has the sample rate and clock source showing before you open Pro Tools.
I can't say I have this problem unless I open pro tools without it on or connected.
