I have more than one version of Pro Tools installed on my Mac. I modified the PT12 icon to show which version of Pro Tools it will launch. Here's a dropbox link to download it if you want to use it on your own system.

To use the icon, follow these steps.
1. Select the Pro Tools app in the finder (in the Applications folder) and press Cmd+i to open the Info panel.
2. Open the icon in Preview. The background should look checkerboarded. If it's not, select Show Image Background in the View Menu. Press Cmd+A (to select it) and then Cmd+C (to copy it).
3. Back in the info panel, click on the icon in the top left corner and press Cmd+V to paste your customized icon image.
