First go to Control Panel>Programs and Features and make certain you have the Eleven Rack Driver installed.

I would suggest you turn OFF the Eleven Rack. Then uninstall the Eleven Rack Driver (if it is installed). Then uninstall ASIO4ALL.
Then
trash preferences
Restart the PC.
Download and install the Eleven Rack driver. The Eleven Rack should be OFF this whole time.
Once the driver is installed, turn OFF the PC.
Turn ON the Eleven Rack, then turn ON the PC.
Try to launch Pro Tools. What happens?
